PROJECT PREVENT COLLISION is a survey that asks individuals to provide information about bird-window collisions in the past and present, and to describe architectural features of the house as well as characteristics of the yard and neighborhood. The survey will take approximately 3 hours of time over a four-week period. To participate in PROJECT PREVENT COLLISION follow these five easy steps: 1) Fill out the Personal Information Form 2) Fill out the General Survey Form 3) Fill out the Past History Survey Form 4) Follow the protocol and fill out the Present Survey Form 5) The four completed forms should be sent to: Dr. David J. If you have more than one window at your house where a bird-window collision has occurred, you will need extra data sheets from either the Past History Survey Form or Present Survey Form. Below are links to extra data sheets.
